10.52.2 Explain the role of human error in aviation accidents.
Human error can play a significant role in aviation accidents.
Pilots, air traffic controllers and maintenance crews are all human and can make mistakes.
Errors in:
- judgement;
- decision-making;
- communication; and
- situational awareness
can contribute to accidents.
There are various factors that can contribute to human error, including:
- fatigue;
- stress;
- distractions;
- lack of experience or training;
- communication breakdowns; and
- inadequate procedures or equipment.
It is important for individuals and organisations to recognise these factors and take steps to reduce their effects.
The aviation industry continually improves:
- training;
- procedures; and
- technology
to minimise human error and enhance safety.
10.52.4 Explain the degree to which human error can be eliminated.
In aviation, the goal is to minimise human error as much as possible.
However, human error cannot be completely eliminated.
The aviation industry therefore uses a range of measures to reduce both the likelihood and consequences of errors.
These include:
- comprehensive training programmes;
- strict adherence to standardised procedures;
- advanced technology; and
- robust safety management systems.
